PHCM is a company which specializes in the management of Healthcare, Senior Communities and Residential / Assisted Living Facilities in California.

A full assessment of the facility is made emphasizing 5 major items:
1. Quality of Care /
Quality of Life:
This assessment is made using the Minimum Data Set (MDS),
CMS's Quality Indicators (QA) for the specific facility (for SNF's)
and the State survey for the last two (2) years. A prioritized list
is created by the PHCM team which includes the goals and objectives
to work on for the following three months.
2. Life and Safety:
An assessment is performed in the facility reviewing the fire
drills, fire exits, extinguishers, generator, fire alarms, and
sprinkler system as well as staff training in the event of an
emergency to ensure that the residents are ALWAYS safe.
3. Services Provided:
This assessment is to ensure that the residents are receiving the
services they are paying for, as well as to evaluate the need of new
services to be offered. This assessment also includes a review of
the facility's staff, qualifications and performance over the last 2
years.
4. Financial:
A full assessment of the facility's current financial status is made
by PHCM. This includes a review of current practices as well as
money management Policies and Procedures. This is made with the
intent of creating new systems that may be more cost effective for
the facility.
5. Marketing:
Determining the source of where the residents are coming from,
as well as who is referring these residents are key factors in
marketing. The PHCM team will assist the current admissions director
in focusing on these areas and create new programs when the Quality of
Care / Quality of Life and New Services provided are improved in the
facility.
